Excerpts from Miami Township Police Reports regarding suicide attempts at KHK:
#01-03-008441 Date: 9-30-03 Time: 1745;
Male student had cut his arm over the past two days with a plastic knife and
a piece of glass. Male does not know why he did it. The cuts were minor and
were cared for by MTFD squad. Male was transported to CMH and RO served the
hospital with a pink slip. A member of the Kids Helping Kids staff responded to
the hospital. No further action needed.

#01-02-013250 Date: 9-13-02 Time: 2345;
Reportee advising of 15 yoa female having suicidal thoughts. Female agreed to
go voluntarily to Clermont Mercy Hospital for evaluation.

#01-02-011912 Date: 8-22-02 Time: 1715
Reportee advised juvenile female was attempting to cut wrists w/a paperclip.
Juvenile's info is on supplement and was saying that she was going to kill
herself etc. Advised if she stayed at KHK she would kill herself. Juvenile was
transported to CMH and given a 72 hr hold by RO. The staff at KHK advised they
would be in touch w/juveniles parents.

Reportee stated a 16 y/o female was hitting her head against the wall and
stating she would kill herself. Female was transported to Clermont Mercy and
signed in.

#01-02-009041 Date: 7-01-02 Time: 2018
Responded for a suicide attempt. Upon arrival, male suspect had cut his arm
with a plastic knife and stated that he wanted to kill himself. MTFD squad
bandaged the male's arm. The male was transported to CMH and pink slipped in.

Miss Smith stated that the subject took a sleeve and tied it around his neck
and tied the other end around a door handle and was trying to hang himself.
Subject was transported to Children's Hospital by some staff members. The
subject's mother and father followed them to the hospital.

#01-00-0014429 Date: 12-14-2000 Time: 1840
Responded for a combative suicidal juvenile. Upon arrival, female was making
threats of suicide. Female was also in possession of a disposable razor.
Female was checked out by squad. Squad refused to transport female due to lack of
medical condition. R/O transported female to CMH. Female's mother checked the
female in. No further action needed. Enroute to CMH-1915. Out at CMH-1940.

14 yr old female threatened suicide. Transported to CMH 72 hour hold placed.

Comp advised that ******* broke out a window picked up a piece of glass, cut
himself then threatened to cut a staff member. Staff and comp declined any
charges transported to CMH by Miami Twp Ems for psych evaluation. 72 hour hold.

RO: Ziegelmeyer, Mark G
#01-00-004035 Date: 4-08-2000 Time: 1430
Dispatched for a suicide attempt and diabetic emergency on a 17 yr old. Upon
arrival met with reportee who advises that juvenile refused to take insulin
and had attempted suicide earlier via metal piece to neck. Also made threat to
kill himself. EMS checked juvenile, all okay. Juvenile told officers that he
wanted to die and wanted to kill himself. We transported juvenile to CMH pink
slip. See suspect arrest sheet for juvenile information.
