BEREA, Ohio -- A 15-year-old boy who died Tuesday following an altercation at a group home was physically restrained before he died, according to police records.

The teen, Kenneth Barkley of Cleveland, died Tuesday night shortly after he became upset with an OhioGuidestone child care worker, punched that worker in the face and was physically restrained, according to a police report.

The Cuyahoga County Medical Examiner's office has not yet released the cause and manner of his death. It is unclear at this time whether the employee's use of physical restraint was a factor in his death.

An OhioGuidestone spokeswoman said Friday that the child care worker is on leave, which is standard protocol for the organization. The spokeswoman said the worker has never had a complaint filed against him and that he has been fully trained in the proper way to restrain a resident.

On Tuesday around 6:43 p.m., a Berea Police Department officer responded to the group home for a report of a boy assaulting a staff member, according to a police report. While en route dispatch advised the officer that the boy was unconscious and not breathing.

An employee led the officer to a rear living room area, where the officer saw Kenneth lying unresponsive on the floor. The officer began CPR and two more officers assisted him when they arrived at the scene. Berea Fire Department paramedics also responded and transported the boy to Southwest General Health Center, where he died.

Officers spoke with two group home employees and four teenage residents, and each offered a similar account of what happened, according to a police report.

Witnesses told police that Kenneth was using a computer in the rear living room area when a male employee told him he needed to take medication. Kenneth declined, and the employee took away the computer keyboard, saying Kenneth wouldn't be allowed to use the computer until he took his medicine.

According to police, witnesses said that at that point, the boy became upset, turned over a table and punched a hole in a wall. The teen then moved toward the employee and began to punch the employee in the face.

Witnesses said the employee restrained the boy in a bear hug and took him to the ground in a "controlled manner," according to the police report. The employee continued to restrain him for a short time afterward, instructing the boy to stop fighting.

The employee eventually stood up and witnesses said Kenneth initially appeared to be "snoring" before they realized he had stopped breathing.

Shortly after the incident took place the worker who had restrained Kenneth became ill and upset, according to a separate police report. An officer attempted to aid the employee to keep the employee from injuring himself; the officer injured his back in the process and received medical treatment.

Alyssa Evanoff, OhioGuidestone's director of communications, said the worker is "distraught" about the incident.

The Berea Police Department and the Cuyahoga County Division of Children and Family Services are investigating the incident. Mary Louise Madigan, a spokeswoman for the DCFS, said Thursday that it had completed its initial review of the incident; however, she declined to reveal the findings of that review, citing privacy laws. She referred to the incident as "a very, very sad situation for everyone involved."

OhioGuidestone said in a statement Wednesday that it is working closely with investigators, but that it could not reveal additional information about Kenneth, citing health care privacy laws.

Employees' use of physical restraint techniques at residential facilities came under scrutiny in 2008 following the death of 17-year-old Faith Finley at Parmadale Family Services. In December 2008, Faith died at the residential facility shortly after being physically restrained by staff.

Three former Parmadale employees were charged with involuntary manslaughter following Faith's death, but they were found not guilty of all charges in February 2010.

In that incident, two women restrained Faith face-down on the floor to control an outburst. A third woman, who was a supervisor, failed to check on Faith for more than an hour, according to prosecutors.

The Cuyahoga County coroner determined the pressure from the restraint and Faith choking on her own vomit killed her.

Evanoff said the employee who restrained Kenneth, like all of OhioGuidestone's employees, received extensive training about the proper way to physically restrain residents.

"All of our group home workers have a very strict training protocol," she said Friday. "Nobody is allowed to work without going through that training."
