Interactive Questionnaire dealie

I think it would be hellah pimp if we could get one of those interactive questionnaires that allow people to answer questions about programs anonymously. I'd like it to look something like this:

I plan to get an account with this company for specific questionnaires, but I think we'd be well advised to get a general survey for our own purposes.

easy as pie for the portal, dunno about implementing that code across all the individual blog sites though... i'd go as far as to say it would be a major pain in the ass for psy, but i'll look into it. ;)

I think the questionnaire is easy enough to do in PHP.  I used to do them so that the answers would then load into excel. It has been awhile, but I could probably try to remember.

I seem to have encountered a similar problem with interactions and 1516 with portico 0.8. The GUI allows me to see that the correct federates are subscribed and publishing the interactions, but when an interaction is sent, it is never received.

Can you advise?
